Other Information
Cancelling Appointments
If you cannot keep your appointment, please inform the reception staff. Failure to cancel an appointment means that other patients who could have had your timeslot if you had cancelled, have to wait longer for an appointment.
Any patient not attending three appointments in a year without cancelling, will receive a warning letter. Any further non-attendance without cancelling can result in the practice asking you to leave the list.
Consultations
Patients Aged 16-75
Patients aged between 16 and 75, who have not been seen at this practice within the last three years, have the right to a consultation with a doctor/health care professional at which time lifestyle inquiries and examinations may be undertaken, as appropriate. Please ask at reception for such an appointment.
Patients Aged 75 And Over
Patients aged 75 years and over, who have not been seen at this practice within the past 12 months, have the right to a consultation with a doctor/health care professional at which time lifestyle inquiries and examinations may be undertaken, as appropriate.
Such a consultation can take place in your home, where in the opinion of the doctor/health care professional, you are unable to attend the surgery because of certain medical conditions.
Patients' Rights and Responsibilities
Patients have the right to be seen by a doctor/health care professional for diagnosis and treatment of their medical conditions. This is obtained by making appointments from the range offered by the practice.
Patients have the responsibility of keeping such an appointment, if at all possible. Patients also have the responsibility of informing the surgery if they are unable to keep their appointment, thus making it available for another patient to book it.
Newsletter
We produce a newsletter every three months for our patients – please collect a copy from the surgery.
Carers/Housebound Patients
If you are looked after by a carer and are housebound, please inform our reception staff to ensure that we hold these details in your medical records.
General Practitioner Registrars
Our practice is a training practice. Our GP Registrars are fully qualified doctors and have a great deal of experience. These Registrars are attached to our practice for six to twelve months. The Registrars usually become General Practitioners after completing their training.
Video Recording
There will be times when our GP Registrars need to make video recordings of their consultations for training purposes. Intimate physical examinations will not be recorded, and the camera will be switched off on request.
The tape is used for the purpose of assessment of the Registrar, for research, learning and teaching purposes. The tape will only be seen by persons who have legal access to your medical records.
The trainer is responsible for the security and confidentiality of the video recording. Should the tape have to leave the practice premises, it will be sent by hand courier or registered post. The recordings will be seen inside the practice, but may need to be seen in another location by personnel appointed to assess the standards of our Registrar.
The tape will be erased as soon as possible but definitely not later than one year after the recording was made. You may be asked when booking an appointment if you wish to partake in this. We hope you will give your permission and be able to help.
